Great Lakes Surgical offers self-pay options for patients who prefer to pay out of pocket, who have high-deductible plans, or Canadian patients who choose our practice. Learn more about our costs and policies here.

Self-Pay Colonoscopy/Upper Gastric Endoscopy

It is the policy of Great Lakes Surgical Associates to collect self-pay fees in advance of your procedure.

Listed below are the fees that you will be charged for services. Please make checks payable to the following:

Great Lakes Surgical Associates: $ 1,500.00

X-Cell Laboratory for Pathology: $120.00
(approximately – if biopsy perform or polyp removed)

As mentioned above, payment is expected in advance of this procedure.

NOTE: Full payment is U.S. dollars (cash or credit card) is required three (3) days prior to your scheduled procedure. If payment is not received within this timeframe, your procedure will be canceled and you will be charged a $50.00 rescheduling fee. Your procedure will be rescheduled at a future time that is convenient for all parties. For all other in-office procedures/therapy, payment in full (U.S. dollars via cash or credit card) is required at check-in. This is a reduced schedule of charges for self-pay patients only. No insurance billing, claim adjudication or financing is offered. If insurance billing, referrals or authorizations are needed, our standard fee schedule and payment policies will apply to all services rendered.
